A formulary represents the prescription therapies believed to be a necessary part of a quality treatment program. Viva Medicare will generally cover the drugs listed in our formulary as long as the drug is medically necessary, the prescription is filled at a Viva Medicare network pharmacy, and other plan rules are followed. The formulary may change during the year. For more information on how to fill your prescriptions, please review your Evidence of Coverage.
Please review the information in the Formulary Introduction to learn more about your Viva Medicare drug benefit including how to ask for an exception if your drug is not on the formulary, has a quantity limit, or is covered as a non-preferred drug.
To use the formulary search tool:
- Click the plan and click to proceed to drug search
- Enter the drug name
- Click the dosage and medication type you are searching for
- Click the Add to List button
The list will display the drugs that are chosen and if the drugs are covered. If it is a covered drug the list will show if it is a generic drug or a brand drug as well as what drug tier it is on. If there are any restrictions (e.g., quantity limits), that will display as well.

You won’t pay more than $35 for a one-month supply of each insulin product covered by our plan, no matter what cost-sharing tier it’s on.
*Members enrolled in Viva Medicare Extra Value and Viva Medicare Extra Care that receive “Extra Help” (limited income subsidy) will pay $0 for insulin covered by our plan.
*Notice for Members of Viva Medicare Select: Your plan does not include prescription drug coverage. Therefore, the formulary and formulary search tools are not applicable to your plan.
